I, (Legal Name) , (The Undersigned), do hereby acknowledge that I am older than eighteen years of age and are knowingly and voluntarily signing this document. I, The Undersigned, WILL live at the Fraternity Shelter starting August 15th 2015 until July 31st 2016. I understand by signing this document I will occupy one (1) bed space at the Fraternity Shelter. I also know and understand all the Shelter Rules and Guidelines, Member Responsibility Guidelines (MRG), Delta Tau Delta Fraternity By Laws and the Theta Gamma By Laws and Standing Rules. I understand that if I violate any of the aforementioned rules or By Laws that I shall be sent to Honor Board and punished accordingly.
I, The Undersigned, understand that by signing this document I am NOT signing a lease with the Fraternity Shelter or its Management Company. I do acknowledge that one (1) bed space shall be reserved for me. If the Fraternity Shelter is not ready for the Fall 2015 Academic Year I WILL NOT be responsible for living at the Fraternity Shelter, the Fraternity, its members, and affiliates ARE NOT liable to find and or acquire alternate housing for me, The Undersigned.
